Infant Tongue Tie

Tongue tie, also known as ankyloglossia, occurs when the tissue between the undersurface of the tongue and the floor of the mouth, called the lingual frenulum, interferes with the normal movement of the tongue. Craniosacral Therapy

CST is a gentle, hands-on approach to healing that focuses on the craniosacral system and the connective tissue in the body. During a craniosacral therapy session, a trained practitioner uses light touch to release tension and improve the function in the body. This can be especially helpful for infants and young children. It has been shown to be helpful for both adults and infants for problems including torticollis, tongue tie, TMJ, pelvic pain and more.

Family Sleep Support

A Sleep Consult is a virtual appointment that is highly individualized to help families find practical and successful solutions to healthful and safe sleep. Juliana Johnson ARNP, IBCLC is a Family Nurse Practitioner and International Board Certified Lactation Consultant with advanced training in newborn and child sleep support. She takes an individualized approach to each family, using holistic methods that are evidence and science based.

Should I feed my baby before my visit?

We recommend bringing your baby hungry. This way we can work on latching at breast or bottle while in our office and observe your baby feeding. That being said, we also understand that timing your baby's feeding for our visit is not always reasonable! If you need to feed your baby prior to your appointment, we recommend giving a "snack" feeding. This way your little one will still likely be willing to feed while in office. 

What can I expect in a typical lactation visit?

In a typical new client lactation visit, one of our experienced Family Nurse Practitioner Lactation Consultants meets with you, getting a detailed history, completing a physical newborn exam and maternal breast exam if appropriate. She obtains an accurate weight of your baby and offers medical recommendations, including labwork (that can be done in our office!) herbs and medications as appropriate. Then, you and your baby meet with one of our Registered Nurse Lactation Consultants and review latch, positioning and other best practices to support your unique concerns or challenges. The visit is typically 60 to 90 minutes. 

Can I use my health insurance for my visit?

Yes! Pacific Wellness is in-network with most major insurers and all WA State medicaid. Please contact your insurer for specific details on the benefits of your individual plan and to confirm the In-Network status of our clinic and providers. You are responsible to know your insurance coverage and benefits before your scheduled visit. Use of insurance does not guarantee full coverage of services. If your individual plan has a deductible that has not yet been met, copays or coinsurance required for services, you will receive a bill for those amounts due. What is tongue tie and how can it affect my baby?

Tongue tie, also known as ankyloglossia, occurs when the tissue between the undersurface of the tongue and the floor of the mouth, called the lingual frenulum, interferes with the normal movement of the tongue. Tongue tie restricts normal range of motion of the tongue and can adversely affect breastfeeding. Tongue tie is associated with persistent pain with latch, slow or poor infant weight gain, reflux and low milk supply.

When indicated, a procedure called frenectomy can be done to correct tongue tie. This is where a CO2 laser is used to remove the tie, releasing the restriction and allowing for more range of motion of the tongue. The procedure is done in our office and babies feed immediately afterwards. Parents and caregivers are instructed on how to perform aftercare tongue stretches and families follow up with a lactation consultant.

FAQs

What is laser frenectomy for infant tongue and lip tie?

Laser frenectomy is a highly focused CO2 laser beam used to release infant oral restriction (tongue & lip tie). This laser vaporizes, cleanly and precisely, while sealing blood vessels at the same time. This procedure removes the restricted infant oral frenulum.The laser frenectomy takes between 5 and 15 seconds to perform, babies and parents are in our laser room for 5 to 10 minutes.
 
In the hands of our experienced providers, the CO2 cold laser provides a safe, precise and often bloodless procedure to release tongue and lip ties and babies feed immediately after. We provide comfort measures in office to reduce pain and make the experience positive for everyone involved. A Registered Nurse Lactation Consultant stays with you and your baby after the procedure to help with latch and develop a feeding plan if needed.
 
In addition to this advanced technique, we incorporate lactation, breastfeeding support and infant feeding support in every visit. We also provide evaluation and same day procedures as we know that appointments with a new baby can be challenging. What can I expect during a visit for laser frenectomy?

If your family has been referred to us for tongue tie you can expect a thorough and gentle evaluation of your infant's mouth and how they are feeding. If your baby is breastfeeding or nursing, this means looking at how your baby is latching. If your baby is bottle feeding, this means looking at how your baby is latching to the bottle. In addition to this thorough exam and because all of our clinical staff members are specialized in complex lactation support, we also have the opportunity to address issues like low milk supply, painful latch, persistent sore nipples, slow infant weight gain and other common concerns surrounding breastfeeding, nursing, pumping and infant feeding. 

After we have completed the exam, we discuss the recommendations and options for you and your infant. This might include a recommendation for frenectomy. This also usually includes working on latch and positioning, care strategies for sore nipples and breasts, bottle feeding and pumping support and a plan to support a healthy milk supply and your infant's weight gain.

If a frenectomy is indicated, we usually have the availability to do it within the appointment that same day. A family member is often welcome to accompany their infant for every part of the procedure, including into the laser room. After the frenectomy a Registered Nurse Lactation Consultant remains with you and your family and helps your baby latch to breast or bottle and makes sure that you and your baby are comfortable, recovered and confident with your plan of care.

You and your baby follow up usually within a few days to a week and sometimes sooner, depending on your care plan.  We provide you with our after hours and on call number should you need any urgent concerns addressed. For more information, check our our Resources for Parents & Professionals. 

What is involved in aftercare for laser frenectomy?

Frenectomy aftercare includes gentle infant oral exercises that promote lots of tongue movement during the healing process. We demonstrate how to do this before the procedure, and encourage families to record the exercises performed with their infant to refer back to. When we see you for your follow up we recheck how your baby's mouth is healing.
